Geriatric Care Managers

It can be overwhelming for caregivers to try to navigate by themselves the numerous resources available to assist with their loved one with dementia.  Caregivers may not even realize what types of services are available.  When additional help is needed, geriatric care professionals can provide this essential support.  The experts working in these roles are often case managers or social workers.  Services which may be available include finding the right memory care or assisted living facility, identifying appropriate health care providers, and helping to arrange for daytime or respite care.  

Geriatric care manager services are usually paid out-of-pocket, but some providers may offer a sliding scale of fees.  PACE (Program for All-inclusive Care for the Elderly) is available particularly for lower income persons.
